How to Enroll in HDAP

If you live in Massachusetts, are HIV-positive, and meet income guidelines (with a gross annual income not exceeding 500% of the federal poverty level, currently $54,150; add $3,740 per dependent), you may be eligible to enroll in HDAP/CHII. U.S. citizenship is not required.

To enroll, complete an HDAP application and mail the completed form with all support documentation to:

Please be sure to fill out the application completely and clearly; we cannot process applications with incomplete or missing information.

From the time of receipt of completed application materials, please allow two weeks for your application to be processed. You will be notified by mail of our decision.

We suggest you make a copy of your application for your records.
